What you should consider while deciding about institutional facility

Note When the family members make the decision to place the patient in an institution they want to make sure that the person is placed in a well respected facility. The points provided below may help you make the decision. The website is not trying to encourage institutionalization in any way.  

1. Does the facility have experience in handling dementia patients?
2. Most important point is that the facility should be closer to caregiver’s residential area so that family members can visit as often as possible. Inquire about visiting hours. Frequent visits by the family members help maintain mental and emotional health of the PwD. It is also possible to get a better understanding of the care situation, and problems faced by the PwD. Understanding problems should be seen as providing help to the care employees and not to raise issues (as far as possible) and provide timely help as and when required. It is important that care staff is receptive to your concerns and suggestions.  Hence it is necessary to maintain good relationship with the staff as far as possible.
3. How is the physical appearance of the facility? Does it feel welcoming, is it clean? Smelly? How are the rooms maintained? How clean is the cooking area? Bathroom? Are measures taken to prevent falls?
4. What is the ratio of resident’s v/s staff?
5. What are the recreational activities? Are activities conducted daily such as physical fitness? Hobbies? Is there any open space? Are group activities conducted? Are natural elements added to the environment such as plants, pets etc.? Are there special celebrations during Diwali etc?
6. Is there a social worker on staff?
7. What is the protocol for handling problems should a PwD face any? With the staff? With fellow residents? With physical illness?
8. Is there a doctor available on the facility? How are regular medical problems handled? How are emergency problems handled?
9. Is there any transportation facility available?
10. Is it possible to get any references? Can the caregiver and PwD stay for a few days on a trial basis?
11. What is the admission requirement? What is the cost? How does it compare with the services provided? What is covered in the monthly expenses? What are additional costs such as for doctor consultation?
12. Who is the administrator (person who overviews the facility)? How often does this person visit the facility?
13. How is staff supervision and training managed? Is there a video camera on the facility recording staff-patient interaction?
14. How are meals served? In a room or in a group setting? Is the food fresh and hot? How is the menu planned? What special considerations are kept in mind while preparing food? For example chewing difficulty, constipation, digestion or acidity problems observed among the elderly persons.

Tip about Managing Pension Accounts

Pension account can be a joint account but only with the spouse. ATM card is available for pension account. Hence withdrawal by person other than the account holder may be possible. Pension account can be linked with the savings account (in the same bank/branch) and provision can be made to transfer money from pension account to the savings account. Caregiver other than spouse could be a joint account holder on the savings account. This gives the authority to sign on behalf of the patient. If the patient cannot sign thumb impression has to given to the bank authorities.

It is also a good idea to get a power of attorney papers done, so that other financial matters could be managed on behalf of the patient.
